with the requested
information.
For application processing to begin, you
MUST
include with the application:
1. Current registration (or documentation) in
applicant’s name.
2. Current insurance in applicant’s name.
3. A current photo of the boat.
4. A current household utility bill in applicant’s name,
showing your current address.
5. Vessels older than 30 years must submit a survey no
more than 3 years old.
